Skilled nursing care is the delivery of daily nursing and rehabilitative services performed by skilled medical personnel. This care is administered with timely and realistic goals set at the start of treatment in collaboration with the physician, resident/family and skilled medical staff. Short and long-term rehabilitation services are prescribed by a physician for those residents who meet rehabilitation criteria for skilled nursing services or for those experiencing serious health issues requiring immediate and daily medical intervention. Skilled care is provided in a nursing home setting by trained nurses and physical/occupational/speech therapists all supervised by a physician. Treatment goals are reviewed regularly by the treatment staff and in cooperation with the resident/family.
Southwind Healthcare and Rehabilitation’s skilled nursing services provide the highest level of care available outside of a hospital setting for individuals who have health conditions that require constant monitoring and active treatment. Nurses and certified nursing aides are available around the clock to provide care for residents recovering from surgery, a hospital stay, an acute or extended illness and those that need long-term supervision and care. The primary goal of our skilled nursing program is to help individuals regain function at the highest level possible and to return home when possible. Our skilled nursing care provides:
